What to Bring to the Appointment:

• A Photo ID: Driving License/Passport
• A list of your current medications and any drug allergies
• Your current insurance card(s)
• Any applicable medical records that you may have.
• Any applicable fees, copayments, or coinsurances.

We highly recommend that you verify your insurance benefits  with your insurance before coming to our clinic.
